The CNA provides personal care and related services in the home. She/he functions under the direction, instruction and supervision of the staff nurse and the Director of Clinical Services and/or appropriate supervisor.

Qualifications:

  • Completion of the 9th grade; prefer high school graduate
  • Ability to read and write & communicate in English consistent with job requirements
  • Completion of a qualified, state approved training program consistent with MA and federal requirements and for which a CNA certificate was obtained
  • Satisfactory performance on a competency evaluation
  • The emotional and mental maturity necessary for establishing and maintaining a good work relationship with the patient, patient’s family, and the personnel of the Agency
  • Reliable means of transportation to get to assigned patient visits
  • Current CPR certification
  • Must have a criminal background check & other checks as required

The Companion is a paraprofessional, providing services in the home for clients under the direct supervision of a qualified supervisor. Companion services include support, encouragement, companionship, respite breaks for family or caregivers, and provision of and instruction in reporting of changes in the client’s situation. The Companion is assigned to a client by the coordinator and follows a written plan of service.

Qualifications:

  • High School graduate
  • At least one (1) year of working as a companion or life experience in a working environment
  • Use of an automobile with possession of liability insurance is desirable
  • Must be able to read, write in English and follow instructions
  • Selected on the basis of such factors as: an interest in people, tact, a history of emotional stability, dependability in employment, neatness in appearance and practices, good personal hygiene, and good judgment
  • Must have a criminal background check
  • Must have current CPR certification

The home health aide provides personal care and related services in the home.

Qualifications:

  • Completion of the 9th grade; prefer high school graduate
  • Ability to read and write in English, consistent with job requirements
  • Completion of a basic aide training program, a minimum 75 hours, consistent with MA and federal requirements and for which a certificate was obtained
  • Satisfactory performance on a competency evaluation and annual reevaluation
  • Has the emotional and mental maturity necessary for establishing and maintaining a good work relationship with the patient, patient’s family, and the personnel of the Agency
  • Has a reliable means of transportation to get to assigned patient visits
  • Must have a criminal background check
  • Must have current CPR certification
  • Prefer 1 year experience as an Aide in a health related setting

A person, who under professional supervision, provides assistance with nutritional and environmental support, personal hygiene, feeding and dressing.

Qualifications:

  • Has successfully completed a MA approved training program (HHA), minimum 75 hour, and has appropriate clinical supervision
  • Must have a criminal background check
  • Must have a current CPR certification
  • Must have a minimum of 1 year experience as an Aide in a health related setting

The homemaker is a paraprofessional employee, trained to provide, home maintenance services for patients under the direct supervision of the nursing supervisor. Homemaking services include dietary management, household services essential to the patient’s health and wellbeing, and provision of and instruction in reporting of signs, symptoms, and/or changes in the patient’s condition.

Qualifications:

  • Successful completion of at least 40 hours of generic training to be completed by the end of the 6th month of employment (can be done by shadowing/precepting) or verification of homemaking services provided in the community equivalent to 40 hours
  • Use of an automobile with possession of liability insurance is desirable
  • Must be able to read, write and follow instructions in English
  • Selected on the basis of such factors as: an interest in people, tact, dependability in employment, neatness in appearance/practices, good personal hygiene, and good judgment
  • One year of work experience
  • Must have a criminal background check

A Licensed Practical Nurse (LPN) provides practical nursing skills within the scope of nursing practice standards in collaboration/under the supervision of the RN case managers/supervisors, teaches and educates patients and their families according to the established professional plan of care.

Qualifications:

  • Graduate of an approved LPN nursing school
  • Currently, licensed “in good standing” in Massachusetts
  • A minimum of one (1) year of experience as a licensed practical nurse, preferably in a home care setting
  • Must have a criminal background check
  • Must have current CPR certification

The MSW provides medical social services within the scope of MSW practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families.

Qualifications:

  • Masters Degree in Social Work from an approved school
  • Graduate of an approved professional School of Social Work approved by the Council of Social Work Education
  • Currently, licensed “in good standing” in Massachusetts
  • Minimum of one (1) year of experience as an MSW, preferably in home care
  • Must have a criminal background check
  • Must have current CPR certification

An Occupational Therapist currently licensed and registered in the state of practice, who provides professional services within the scope of occupational therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families. Occupational Therapy in the home is performed under the supervision of the RN case manager, Administrator, Director of Clinical Services. If Nursing has opened the case, Occupational Therapy is a secondary discipline and therefore works collaboratively and professionally with the RN case manager to provide necessary services.

Qualifications:

  • Graduate of an approved professional Occupational Therapy program
  • Currently, licensed “in good standing” in Massachusetts
  • Minimum of 2 years of experience as an occupational therapist, preferably in home care
  • Must have a criminal background check
  • Must have current CPR certification

An Occupational Therapist Assistant provides professional services within the scope of occupational therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families.

Qualifications:

  • Graduate of an approved professional Occupational Therapy Assistant program
  • Current licensed “in good standing” in Massachusetts
  • Minimum of 2 years experience as an OTA, preferably in home care
  • Must have criminal background check
  • Must have current CPR certification

A person, who under professional supervision, provides assistance with nutritional and environmental support, personal hygiene, feeding and dressing.

Qualifications:

  • Successfully completed a MA approved training program or a 60 hour training equivalent in PCA skills
  • Shows an interest and concern for people, good judgment and tact in dealing with the sick
  • Practices good hygiene and is neat in personal appearance
  • Speaks, reads, and writes in English
  • Must have current CPR certification
  • Must have criminal background check

A Physical Therapist provides professional services within the scope of physical therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families.

Qualifications:

  • Graduate of an approved professional Physical Therapy program
  • Currently, licensed “in good standing” in Massachusetts
  • Minimum of 2 years of experience as a physical therapist, preferably in home care
  • Must have criminal background check
  • Must have current CPR certification

Under the supervision of the Physical Therapist and according to established treatment plans and specific procedures, provides treatment to physical therapy patients in order to assist them in regaining physical mobility and function, increase strength, and decrease pain.

Qualifications:

  • Completion of an accredited Physical Therapy Assistant Program
  • 2 years experience as a PTA, preferably in home care
  • Current licensure in Massachusetts as a Physical Therapy Assistant
  • Must have a criminal background check
  • Must have current CPR certification

A Registered Nurse provides professional services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates patients and their families.

Qualifications:

  • Graduate of an approved professional nursing program (RN)
  • Currently, licensed “in good standing” in Massachusetts
  • Minimum of one (1) year of experience as a professional nurse, preferably in home care
  • Must have a criminal background check
  • Must have current CPR certification

A Speech Pathologist/Audiologist provides professional services within the scope of Speech Pathologist/Audiologist practice standards in collaboration with primary care physicians, teaches and educates patients and their families, and evaluates and assesses patient’s therapy needs.

Qualifications:

  • A Masters or doctoral degree in speech-language pathology and is licensed as a speech- language pathologist by the state. Or
  • Successful completion of 350 clock hours of supervised clinical practicum (or in the process of completing) AND has at least nine (9) months of supervised full-time speech-language pathology experience AND has successfully completed a national examination approved by the US Secretary
  • Must have a criminal background check
  • Must have current CPR certification
